Fisiologia da Pancreatite Aguda

Resumo:

A pancreatite é caracterizada por uma doença infl amatória do pâncreas, causada, dentre outros fatores, pela liberação prematura de enzimas digestivas, afetando o parênquima deste órgão. Os principais fatores etiológicos da pancreatite são os cálculos biliares e o consumo de álcool. O cigarro, entretanto, o diabetes do tipo II, alguns medicamentos ou doenças genéticas, bem como procedimentos como a colangiopancreotografi a podem também aumentar a frequência desta condição. A pancreatite se manifesta de duas formas uma aguda e outra crônica (LANKISCH et al., 2015).Na sua forma aguda, foi responsável no Brasil por mais de 24.908 internações hospitalares em 2011, em que 5,91% vieram a óbito, apresentando gastos em torno de 16 milhões neste ano (IBGE, 2013). A pancreatite aguda é geralmente leve e autolimitada, no entanto, cerca de 20% dos pacientes podem apresentar complicações locais, como necrose e infecção sistêmica através do desenvolvimento da síndrome de disfunção múltipla de órgãos, o que caracteriza a forma grave, ou necrozante da doença. Dados do governo americano de 2009 mostram um aumento do índice de pancreatite aguda que passaram de 13 para 45 por 100.000 habitantes acarretando elevados custos hospitalares (PEERY et al., 2012).
